Data Protection / Declaration of Consent

With your consent, LEAD Horizon GmbH, Sandwirtgasse 12/1, 1060 Vienna processes your name as well as your gender, your address, your date of birth, your national insurance number, ID card data, your e-mail address and telephone number, the COVID-19 infection status and technical telemetry data, such as your IP address, which are necessary for the operation of the web app and the performance of the tests. In addition, photos of you and your ID card are taken and processed, which are used to establish your identity and for quality assurance purposes. Furthermore, your data will be used for the purpose of creating anonymous statistical evaluations. The COVID-19 test cannot be carried out without your consent.

The photographs of you, which are taken as part of your belt test, are used to ensure the correct application of the test and are only viewed by selected employees at LEAD Horizon and in the event of a complaint and randomly for quality assurance purposes. The photographs of your ID document are read with the help of text recognition software and the contents of the ID documents (document number, expiry date, name) are subsequently processed.

The web app also uses cookies. Only technically necessary cookies are used: lead_horizon_testkit_session - The session cookie is used to recognise you for the duration of your session and is necessary to ensure the functionality of the app. As soon as you close the WebApp, the session cookie is automatically deleted.

XSRF-TOKEN - supports a security measure to prevent cross-site request forgery or cross-site scripting. This cookie is also deleted at the end of your session. lh_id_set - encrypted storage of your sample number when retrieving the result.

Your data will be electronically transmitted to an available laboratory assigned to your region for the purpose of COVID-19 testing. In the case of the "Alles gurgelt" campaign, this is lifebrain COVID Labor GmbH at Wipplingerstraße 35/10, 1010 Vienna.

With your consent, you also agree to the laboratory reporting the finding back to LEAD Horizon in electronic form. In the event of a positive status, the laboratory is required by law to report the infection to the relevant health authority.

Your data will not be transferred to any other third parties or to countries outside the European Union. Exceptions to this are the transfer to order processors such as the hoster Hetzner Online GmbH (Industriestr. 25, 91710 Gunzenhausen, Germany), which operates an ISO-certified data centre in Germany, and Anyline GmbH Zirkusgasse 13 / 2b, 1020 Vienna, which provides the text recognition software, which work exclusively on the instructions of LEAD Horizon, do not use the data for their own purposes and are bound by their own agreements to the data protection obligations of the General Data Protection Regulation.

If you create a user account, only your access data will be kept for 1 year. All other data will be deleted 14 days after the result is sent to you.

Consent is voluntary. Please note, however, that deletion, restriction and revocation mean that the result of the testing can no longer be communicated to you.

You have a right to information about the personal data processed by you, correction and deletion, restriction of processing as well as a right to data portability and a right of complaint to the data protection authority. You can also revoke this consent at any time with effect for the future (e.g. by e-mail to datenschutz@lead-horizon.com).
